Please familiarize yourself with your benefits, and know your insurance coverage. All plans now cover the office preventative medicine visit which covers pelvic, breast, and pap smear exam.
We additionally dip your urine to screen for protein and sugar, and perform a hemoglobin test; an indicator for anemia. Some plans may not cover these two labs. If you want to decline these tests you have the right to do so if you know they aren’t covered. We will always have them as a part of our annual exam because it is good medicine and recommended for prevention.
Below are the two codes and charges we bill for both. You can use these CPT codes to call your insurance and see if they cover them. If they are not covered we will bill you according to how your insurance processes your claim. If you do not agree with their decision please call your insurance and dispute this with them; please refrain from calling our office.
The codes are as follows:
81002 Urinalysis $10.00
(We dip your urine sample in a cup)
88738 Hemoglobin $15.00
(This is when we put your finger in a finger clip and it reads your blood count)
